코로나19 팬데믹 위기의 확산으로 전염병 예방과 관리 및 질병 치료를 위한 공공의료의 중요성이 날로 커지고 있다. 이 연구에서는 제도분석틀(IAD)을 통해 문재인정부에서 의료공공성 강화와 의료개혁 차원에서 추진한 공공의대 설립 정책이 왜 좌절되었는지와 관련하여 공공의대 설립을 둘러싼 구조적 제약 요인과 정책갈등의 메커니즘을 분석하고자 한다. 사례분석을 위해 사회생태적 시스템(SES)을 결합한 수정 IAD 모형(물리적 조건, 공동체 속성, 다양한 수준의 규칙들과 행위상황에 미치는 자원시스템, 거버넌스시스템, 자원구조와 행위자, 상호작용과 정책산출)을 제시하고, 연구의 분석모형에 따라 공공의대 설립을 둘러싼 의-정 갈등의 제도적 구조와 메커니즘을 살펴보고자 한다. 분석결과, 민간 주도의 보건의료체계의 구조적 제약 하에서 보건의료 자원과 거버넌스 구조의 특성 및 행위자의 상이한 선호와 인식차이를 고려하지 않고 정부가 밀어붙이기식으로 공공의대 설립을 추진하였기 때문에 정책 갈등이 나타났음을 알 수 있었다. 이 연구의 함의는 이론적 측면에서 정책 갈등 사례분석을 위한 설명모형으로 IAD의 유용성을 확인하고, IAD 모형 수정을 통해 IAD의 이론적 진화에 기여함을 들 수 있다. 정책적 측면에서는 정보의 비대칭성이 존재하는 자원의 특성과 민간 주도의 보건의료체계의 구조적 제약 하에서 의료계의 저항이 극심한 의료개혁정책을 정부가 밀어붙이기식으로 추진할 경우 성공하기 어려운 측면이 있음을 정책 구조와 그 메커니즘을 통해 확인하였다는 함의가 있다.

With the spread of the COVID-19 pandemic crisis, the importance of public health care for the prevention and management of infectious diseases and treatment of diseases is increasing day by day. This study analyze the structural constraints surrounding the establishment of public medical schools and the mechanism of policy conflicts were investigated in relation to why the policy to establish public medical schools promoted by the Moon Jae-in government in terms of medical reform to strengthen medical publicity was frustrated, through the Institutional Analysis and Development Framework (IAD). To examine the institutional structure and mechanism of the agenda-setting conflict surrounding the establishment of a public medical school through a modified IAD model that comprehensively explains the contextual conditions of the behavioral situation for case analysis and combines the socio-ecological system (SES) to specify the behavioral situation. system, governance system, resource structure and actors, interaction and policy making. As a result of the analysis, it was found that policy conflicts appeared because the government pushed ahead with the establishment of public medical schools without considering the characteristics of health care resources and governance structures and the different preferences and perceptions of actors under the structural constraints of the private health care system.
